Signify Health, a portfolio company of New Mountain Capital, has acquired Triple Aim Ventures (d.b.a. TAVHealth). The acquisition integrates TAVHealth's curated network of community-based organizations and its TAVConnect platform into Signify's technology-enabled in-home care and complex care management services to better identify and manage Social Determinants of Health (SDoH).
